The Community Health Project is a partnership between CDPHP® and CEO. Using a positive approach to health care, we will:
Offer health care support and education to help you make healthier choices for you and your family.
Provide guidance on health care benefits to help you get the most out of your health insurance.
Make it easier for you and your family to access the right health care professionals in your area.
CEO can provide you with the health care connections you need to stay healthy. We will also work with CDPHP to help you manage health challenges that may affect you or your family.
We can offer education and support on a variety of topics, including:
Raising children with asthma
Eating well on a budget
High blood pressure
Connecting with your primary care physician/doctor
Access to CEO’s many services, which can help make your life happier and healthier

To be eligible, you must have CDPHP/Medicaid insurance and be a resident of Rensselaer County.
